When you receive an error that states "Task # is no longer passing", it (almost) always means that you have introduced a syntax error in the current task you are working on that has broken the code checker. Never click on "Go to Task #"... look though the current code to find the syntax error.
Here, if you look at the code you posted, a quotation mark is marked with syntax highlighting to indicate an error. Working back from this (as this usually indicates where the error was detected, not occuring) you should see that you are missing a closing quotation mark in this line:
<select id="size" name="shirt_size>
"shirt_size" is missing the closing quotation mark.
Fix that up and you should be all good.
