HTML HTML Forms Choosing Options Create Radio Buttons

I dont understand what the question is asking me to do?

Where do i have to add the labels? Ive already tried to add them before the input radio sections but that didn't work

There is one main label not associated with any elements then each radio button should have its own label similar to the example seen here
