First, you're going to want to remove your bottom select tag. The top part of the form code should look like this:

<form action="index.html" method="post">
      <h1>Shirt Order Form</h1>
       <label for="color">Shirt Color</label>

Remeber, that a label is a form element in itself. Here we're telling it to add a label of "Shirt Color" to the element that has an id="color" by using the for attribute. Also, don't forget to put in a close your select tag immediately before closing the form.
