Form to Custom PDF

Hello,

I'm looking to make an iOS App that would include a simple "survey" or "form" that someone would fill out with things like Name, Address, Date of Birth, etc.

I would like the app to store that data and use it to fill a PDF application we already have created.

Is this something that is possible? What direction can you point me in?
